A 75-year-old man comes with a 10-minute history of crushing substernal chest pain radiating to his left arm. His ECG shows T wave inversion in lead V3-V4. His creatine phosphokinase level is elevated and his troponin T level is 0.4 ng/mL (normal <0.04). Which of the following is the most likely diagnosis?

الإجابة: A. Acute myocardial infarction
Ischaemic chest pain with T-wave inversion and raised troponin T (0.4, 10x normal) and CK means myocardial necrosis = acute myocardial infarction (NSTEMI pattern). Unstable angina has the same ECG changes but negative troponin; stable and Prinzmetal angina are ischaemia without infarction (no biomarker rise).

-
A
ليش صح
Raised troponin and CK-MB show tissue destruction (infarction); with T-wave inversion this is NSTEMI-type acute MI.

B
ليش غلط
Prinzmetal angina is vasospastic ischaemia with no infarction, so troponin and CK-MB stay negative; its hallmark is transient ST elevation.

C
ليش غلط
Stable angina has ischaemia but no infarction, so troponin and CK-MB are negative.

D
ليش غلط
Unstable angina can show the same ST depression/T-wave inversion but has negative troponin and CK-MB because there is no infarction.
