الإجابة: D. warfarin should be stopped for 5 days, and heparin should be started for bridging.

Before a major operation such as right hemicolectomy, warfarin is held until the INR normalizes (usually 3-5 days), and in a high-thrombotic-risk patient like one with a metallic mitral valve, anticoagulation is maintained with heparin bridging. Simply stopping warfarin (A) leaves the valve unprotected; continuing it (C) or aiming for INR 1.5 (B) risks bleeding; clopidogrel (E) does not protect a mechanical valve.
