A 79-year-old female patient presented to the ER severe abdominal pain. He is drowsy, pulse 127 beats/min, blood pressure is 91/59 mmHg. He has very tender abdomen with tender, red and irreducible inguinal mass. You suspect strangulated femoral hernia. Her ABG as follow: pH 7.1, PaO2 90 mmHg, PaCO2 25 mmHg. HCO3 10 mmHg and base deficit of -10 mmol/L. What is the likely acid-base abnormality, if there is one?

الإجابة: A. Metabolic acidosis

✓ مطابق للمصدر Step Up To Medicine 6th Edition 2024 (Acid-base imbalance).pdf · p. 6

pH 7.1 with HCO3 10 and a base deficit is metabolic acidosis (lactic acidosis from a strangulated bowel and shock). Winter formula: expected PaCO2 = 1.5x10 + 8 +/- 2 = 21–25. The measured 25 falls within this range, so this is a simple metabolic acidosis with appropriate respiratory compensation. The low PaCO2 is compensation, not a primary respiratory alkalosis.
