الإجابة: D. Urgent surgical debridement
Rapidly spreading, very tense and tender thigh swelling with sepsis (fever, tachycardia, hypotension) in an uncontrolled diabetic is necrotizing soft tissue infection. It needs urgent aggressive surgical debridement plus IV broad-spectrum antibiotics and supportive care. Oral antibiotics, antihistamines or an endocrine consult alone would delay life-saving surgery.
