A 36-year-old woman was ejected from the car during a head-on collision with a truck. On arrival to the ER, she was cold and clammy peripherally with decreased urine output. Her pulse rate was 120 beats/min and her blood pressure was 80/60 mmHg. She had engorged neck veins. Her chest was clear with equal and good air entry on both sides on auscultation. What is the most likely cause of her shock?

الإجابة: C. Cardiac tamponade
Shock with engorged neck veins but a clear chest with equal air entry = cardiac tamponade, which impairs filling of the right ventricle. Tension pneumothorax also distends the neck veins, but it gives absent breath sounds and tracheal deviation. A hemothorax reduces air entry and does not raise the JVP.
