الإجابة: C. Place a needle in second left inter costal space.
Distended neck veins, tracheal shift away from the left, absent left breath sounds and hypotension = left tension pneumothorax, a clinical diagnosis. Do needle decompression in the 2nd intercostal space (midclavicular line) immediately, without waiting for a CXR. A chest tube follows the needle; it is not the first step.
