A diabetic patient with fever, marked leukocytosis, and a red, tender leg with hemorrhagic bullae after a puncture wound has a necrotizing soft tissue infection (necrotizing fasciitis). Treatment is urgent, aggressive surgical debridement plus broad-spectrum antibiotics and support. Irrigation, or limited I&D with re-evaluation, delays definitive treatment. Hyperbaric oxygen is not the primary therapy.
