Ischaemic (arterial) ulcers are punched-out, painful and dry with little granulation tissue, so a poorly granulating floor suggests ischaemia.
- A painless ulcer with palpable pedal pulses suggests a neuropathic ulcer; raised everted edges suggest malignancy; purulent discharge indicates infection, not ischaemia.
