A long-standing groin lump in a man that disappears on lying down and enlarges with straining (raised intra-abdominal pressure) is a reducible inguinal hernia, the commonest groin hernia. A saphena varix (A) can also empty on lying down and has a cough thrill, but it sits below the inguinal ligament and the stem gives no venous features. Lymphadenitis (C) and psoas abscess (E) do not reduce. A femoral aneurysm (B) is pulsatile.
