A 55-year-old with epigastric pain, poor intake and iron-deficiency (microcytic hypochromic) anemia must be investigated for upper GI malignancy; anemia is the most common presenting feature of gastric cancer, and upper GI endoscopy with biopsy is the best diagnostic test.
- Starting iron/folate first would treat the symptom and could delay diagnosing a cancer.
- CT/MRI are for staging after the diagnosis is made.
