A 50-year-old woman has colicky right upper quadrant pain for 5 days. She has nausea, but no vomiting or diarrhea. She is afebrile. There is marked tenderness of the right upper quadrant. The liver span is normal. An abdominal ultrasound shows calculi within the lumen of the gallbladder, and the gallbladder wall appears thickened. Intrahepatic and extrahepatic bile ducts appear normal. Which of the following mechanisms is most likely to play the greatest role in development of this disease?

الإجابة: B. Biliary hypersecretion of cholesterol
This is gallstone disease (calculi with a thickened gallbladder wall, normal ducts) in a middle-aged woman, and the commonest stones are cholesterol stones. They form when bile becomes supersaturated with cholesterol (raised cholesterol relative to bile salts/lecithin), so cholesterol hypersecretion is the key mechanism.
- Antibody-mediated RBC lysis (hemolysis) causes pigment stones, which are less common and fit a hemolytic patient, not this one.
- Phosphate excretion and HBV have no role in gallstone formation.
- The old bank had no answer key.
