Shoulder-tip pain from the gallbladder is referred through the phrenic nerve (C3–C5, mainly C4). Inflamed gallbladder irritates the overlying diaphragmatic and parietal peritoneum, and C4 also supplies the skin of the shoulder.
- The vagus does not carry this somatic referred pain.
- Intercostal nerves give local abdominal wall pain, not shoulder pain.
