A tender, globular RUQ swelling that moves with respiration is a distended, inflamed gallbladder. Gallbladder empyema is a distended pus-filled gallbladder after cystic duct obstruction, and an acutely inflamed gallbladder is painful and palpable in about a third of cases.
- Choledocholithiasis: the gallbladder is usually not palpable (scarred by previous cholecystitis).
- Choledochal cyst is a congenital duct dilatation, mostly diagnosed in childhood, and acute pancreatitis does not produce a palpable gallbladder.
