An older patient with progressive dysphagia and marked weight loss should be suspected of having esophageal carcinoma. On barium swallow, cancer shows an irregular narrowing, shouldering or an irregular filling defect. Achalasia gives a smooth, tapering bird's beak instead, and a diverticulum appears as a pouch.
