Neoadjuvant (pre-operative) chemoradiotherapy in rectal cancer shrinks and downstages the tumour. This allows sphincter-preserving surgery, avoids a stoma and improves recurrence and survival. It does not remove the need for adjuvant treatment, does not protect the pelvic autonomic nerves, and is not primarily aimed at liver metastases.
