A 55-year-old gentleman presents with rectal bleeding. On further questioning, the bleeding appeared dark, mixed with the motions and is associated with mucus discharge. He reports no changes in bowel habit and his weight and appetite remain unchanged. Abdominal findings are unremarkable, and proctoscopy showed only first-degree hemorrhoids. الإجابة: D. Colonoscopy should be requested
Dark blood mixed with stool and mucus in a 55-year-old is a red flag for colorectal cancer, and first-degree haemorrhoids do not explain it. CRC must be considered in every patient with rectal bleeding even if haemorrhoids are present, so colonoscopy is required. Reassurance or repeat proctoscopy misses a proximal lesion; FOBT adds nothing when there is visible bleeding.
