الإجابة: B. Urgent laparotomy and resection
Free fluid, free air bubbles around a thickened sigmoid and generalized guarding mean perforated (complicated) diverticulitis with peritonitis. The notes say complicated diverticulitis needs surgery and emergency colectomy is done for generalized peritonitis.
- NPO, IV antibiotics and observation is for diverticulitis without perforation or peritonitis.
- Endoscopy and barium enema are avoided in acute diverticulitis because they can cause or worsen perforation.
