CT abdomen and pelvis with IV contrast is the investigation of choice in acute diverticulitis to define severity and complications (abscess, perforation). Colonoscopy and barium enema are avoided in the acute phase because of perforation risk. A plain film only rules out free air or obstruction, and laparoscopy is not a diagnostic first step.
