Dysuria, pyuria and pneumaturia are the classic triad of a colo-vesical fistula, a recognized complication of diverticulitis; long-standing LIF pain with episodes of diarrhea fits recurrent sigmoid diverticulitis. Crohn's ileitis would give right-sided symptoms, an anal fistula does not reach the bladder, and BPH with UTI does not cause pneumaturia.
