Colicky pain, vomiting, constipation and distension = mechanical bowel obstruction, and a previous laparotomy points to adhesions, the commonest cause of SBO in adults. An operative scar on inspection suggests adhesive obstruction. Volvulus and intussusception are far less common in an adult with a surgical history, and paralytic ileus gives a silent abdomen rather than active bowel sounds.
