الإجابة: A. Abdominal ultrasound
RUQ pain with tenderness and guarding, fever, vomiting and leukocytosis in a middle-aged woman suggest acute cholecystitis, which is gallstone-related. The first test for suspected gallstone disease is an RUQ ultrasound. An erect chest X-ray looks for free air from a perforation, abdominal X-rays look for obstruction, and endoscopy does not assess the gallbladder.
