الإجابة: D. Transfer the patient to the OR for appendectomy
This young male has a typical, high-probability picture (localized RIF tenderness, guarding, rebound, fever, WBC 15,000). The definitive treatment is appendectomy, so transfer him to the OR. NPO, IV fluids and IV antibiotics are given as preparation for surgery, but 'admit and observe' (B) wrongly makes observation the plan and delays surgery. CT is unnecessary when the picture is typical, and ultrasound for mesenteric lymphadenitis fits children, not this case.
