A child with RLQ pain and a recent sore throat (URTI) most likely has mesenteric lymphadenitis: enlarged, tender mesenteric lymph nodes after a viral illness. This is the classic mimic of appendicitis in children. Intussusception affects infants, Crohn's disease gives a longer history with diarrhoea, and renal colic gives colicky flank pain.
