الإجابة: A. Admission, IV antibiotics and observation
A 5-day history with an appendicular mass on ultrasound calls for conservative treatment first: admit the patient, give IV antibiotics and fluids, and observe closely with repeated checks of vital signs, pain and the size of the mass. Immediate surgery (B, D) is relatively contraindicated because the tissues are friable and the bowel is easily injured. Interval appendectomy is done weeks later, not after 5 days (C). Urgent surgery is needed only if the patient fails to improve.
