Midline pits in the sacrococcygeal area with discharge, and hair coming out of one of them, are classic for pilonidal sinus. Hair penetrates the subcutaneous tissue and causes a foreign-body reaction. Anal fistula, abscess and fissure are located at the anus, not over the sacrum and coccyx.
