Painless bright-red bleeding in a young patient with chronic constipation and straining (a risk factor) is typical of internal hemorrhoids. An anal fissure also follows constipation, but it causes sharp pain during and after defecation, so painless bleeding does not fit. Rectal cancer is unlikely at 30 with stable weight, normal appetite and no family history.
