A 2nd-degree internal hemorrhoid (prolapses and reduces spontaneously) that is symptomatic is treated with conservative measures plus a minimally invasive office procedure, and rubber band ligation is the most common one for grade II–III. Total hemorrhoidectomy is kept for grade III/IV, recurrent, or failed banding. Botulinum toxin is a treatment for anal fissure, not hemorrhoids.
