الإجابة: A. Chronic anal fissure- Lateral internal sphincterotomy
Six months of symptoms with a sentinel skin tag and hypertrophied/fibrosed sphincter fibres mean a chronic fissure, so C and D are wrong. The surgical treatment is lateral internal sphincterotomy, which divides part of the involuntary internal sphincter whose spasm stops healing. The external sphincter (B) is never divided because that causes incontinence.
