الإجابة: B. Abdominal US

Five days of pain and fever with fullness in the RIF and high WBC suggests an appendicular mass/abscess. Ultrasound is the useful initial imaging here: it helps exclude an appendicular mass or abscess and confirms an abscess by showing fluid in the mass. A plain abdominal X-ray (the old bank key) does not show this, so the bank answer C is not supported.
