الإجابة: B. CT abdomen with PO and IV contrast
LLQ pain, fever, marked leukocytosis and LLQ fullness suggest complicated diverticulitis (possible abscess). With no free air or generalized peritonitis, the next step is a CT scan to define the extent of disease; an abscess seen on CT is drained. Immediate laparotomy is for perforation/peritonitis or refractory disease, and colonoscopy is avoided in the acute phase.
