Fever with LLQ tenderness in a 50-year-old suggests acute diverticulitis. After a plain film (KUB) to exclude free air, which isn't one of the options, the diagnostic test is a CT scan to show the extent of disease (abscess, perforation). Colonoscopy/sigmoidoscopy are avoided in the acute phase because of perforation risk.
