Isolated thrombocytopenia (20,000) with bruising, normal Hb/WBC and HIV/HCV excluded means primary ITP. Treatment is indicated because the platelets are <30,000, and the first-line therapy is corticosteroids (or IVIG).

- Splenectomy and TPO-receptor agonists (eltrombopag) are second-line options.
