on observation, a patient has a left facial droop. On closer examination, his nasolabial fold is flattened. When asked to smile, the left corner of his mouth droops. He is unable to keep his cheeks puffed out. Eye closure is only slightly weaker compared to the right. His forehead wrinkles when he is asked to look up high. What is the diagnosis?

The left lower face is weak but forehead wrinkling is preserved and eye closure is nearly normal. This is an upper motor neuron (central) facial palsy, so the lesion is contralateral (right hemisphere).
- Bell's palsy (E) is a lower motor neuron lesion that weakens the upper and lower face on the same side.
- A right MCA stroke causes left-sided weakness of the face and arm more than the leg. That fits a predominantly facial deficit, so D is the usual key.
- A right internal capsule lesion (B) also gives a left UMN facial palsy, but typically with face = arm = leg involvement. The sources do not fully separate B from D for an isolated facial finding.
