A homonymous hemianopia with macular sparing localises to the visual (occipital) cortex, typically a PCA infarct. Visual-field defects are contralateral, so a left hemianopia means a right occipital lesion.
- Optic radiation (temporal/parietal) lesions in MCA strokes give hemianopia/quadrantanopia without macular sparing.
- A left occipital lesion would cause a right-sided field defect.
