الإجابة: C. IV unfractionated heparin

✓ مطابق للمصدر Year4C- Anticoagulants🍓.pdf · p. 9

This post-operative PE patient has significant renal impairment (creatinine 3 mg/dL) and a recent operation. IV unfractionated heparin needs no renal dose adjustment, is given as an IV infusion for therapeutic dosing, and has a short half-life that protamine can reverse if bleeding occurs. Fondaparinux is not recommended when CrCl <30, LMWH is cleared by the kidneys, and subcutaneous UFH is the prophylactic route, not treatment.
