الإجابة: A. Hemolytic uremic syndrome
A young patient with haemolytic anaemia (reticulocytes 10%), thrombocytopenia and acute kidney injury (high creatinine and urea), with a normal INR and no neurological features, fits haemolytic uraemic syndrome. DIC would prolong the INR, and the absence of neurological signs favours HUS over TTP. Chronic renal failure would not explain the acute haemolysis and low platelets.
