Macrocytic anaemia (MCV 108) with a low B12 after gastrectomy (loss of parietal cells and intrinsic factor) plus neurological features (ataxia, paraesthesia) needs intramuscular vitamin B12. Parenteral replacement is indicated both for malabsorption and for severe or neurological deficiency.
- Folate alone can correct the anaemia but won't prevent or treat the neurological damage.
- Iron does not fit a macrocytic anaemia.
- Transfusion is not needed in a stable patient.
