Anaemia of CKD comes from erythropoietin deficiency. With Hb <10, no bleeding and adequate iron stores, the treatment is an erythropoiesis-stimulating agent (EPO).
- IV iron is given when iron deficiency is present. Here stores are already adequate.
- Transfusion is not first line in a stable, non-bleeding patient: EPO reduces the need for transfusions.
- Simply repeating the Hb delays treatment of an established indication.
