Sudden drop in Hb with abdominal pain, splenomegaly and hypotension is a splenic sequestration crisis: blood pools rapidly in the spleen, causing hypovolaemic shock. The marrow is still working, so reticulocytes stay high.
- Aplastic crisis has a low reticulocyte count and no acute splenic enlargement.
- A haemolytic crisis does not explain the painful enlarging spleen and shock.
- Iron deficiency is not an acute event.
