Fatigable weakness worse through the day with bilateral ptosis and weak eye movements = myasthenia gravis, which preferentially affects ocular, facial and bulbar muscles. Bulbar weakness causes slurred speech and difficulty chewing/swallowing. MG affects skeletal muscle only, so paralytic ileus (smooth muscle) and cardiomyopathy are not features.
