Microcytic anaemia with koilonychia and menorrhagia is iron deficiency, and oral iron (ferrous sulfate) is first line.
- IV iron is kept for patients who cannot tolerate oral iron, have a malabsorption disorder (IBD, gastric bypass) or severe IDA. An over-the-counter antacid only reduces absorption: it can be stopped or taken at a different time from the iron, which does not by itself justify IV iron.
- Transfusion is not indicated at Hb 8.5 without cardiopulmonary compromise.
- B12 is irrelevant, as levels are normal and the anaemia is microcytic.
