The picture points to anaemia of chronic disease: months of illness after an infection, weight loss, splenomegaly and a raised CRP suggest ongoing inflammation, infection or malignancy. ACD can become microcytic in its later phase, and inflammation explains the reactive thrombocytosis.
- Iron deficiency (A) is the main alternative: it is also microcytic and also causes reactive thrombocytosis. The stem gives no ferritin, which is the test that separates the two (high/normal in ACD, low in IDA).
- B12/folate deficiency would be macrocytic.
