Fever + confusion + thrombocytopenia + MAHA (schistocytes, high LDH and indirect bilirubin, low haptoglobin) is TTP. Renal impairment is often absent in TTP; only a minority of patients have all five features.
- HUS is defined by impaired renal function (here renal function is normal).
- ITP has no haemolysis or schistocytes.
- DIC is a consumptive coagulopathy with prolonged clotting times in an ill patient (sepsis, trauma, APL).
