An isolated, very high platelet count (1,350,000/µL, far above 600,000) with normal Hb and WBC in an older adult = essential thrombocythaemia. ET is a myeloproliferative neoplasm (JAK2/CALR/MPL) and a diagnosis of exclusion after reactive causes. Vasomotor symptoms such as dizziness or headache are typical.
- Hodgkin lymphoma, amyloidosis and haemolytic anaemia do not cause an isolated platelet count above 1 million with a normal Hb.
