A 32-year-old male presents to the clinic with swelling in his legs and face. He reports dark, cola- colored urine over the past week. Physical examination reveals hypertension and periorbital edema. Laboratory investigations show elevated serum creatinine, proteinuria, and hematuria. A throat culture performed two weeks ago was positive for streptococcal infection. الإجابة: B. Poststreptoccal glomerulonephritis
Nephritic picture (haematuria/cola urine, hypertension, periorbital oedema, rising creatinine) about 2 weeks after a group A streptococcal pharyngitis = post-streptococcal glomerulonephritis (immune-complex, low C3, raised ASO/anti-DNase B).
- Minimal change and membranous nephropathy are nephrotic (heavy proteinuria, no haematuria/HTN picture).
- Diabetic nephropathy needs long-standing diabetes.

A
ليش غلط
Diabetic nephropathy is kidney damage caused by type 1 or 2 diabetes and is mostly asymptomatic (micro- or macroalbuminuria); this patient has no diabetes, and the picture follows a streptococcal infection.

B
ليش صح
PSGN appears about 2 weeks after group A strep pharyngitis or impetigo: immune complexes activate complement, with raised anti-streptolysin-O and anti-DNase B.

C
ليش غلط
Minimal change disease is a nephrotic disease (T-cell cytokines, podocyte effacement, selective proteinuria) seen in children, not a post-strep nephritic picture.

D
ليش غلط
Membranous nephropathy is a nephrotic disease (subepithelial immune deposits, nonselective proteinuria); the hematuria with hypertension after strep is nephritic.
