A 69-year-old man presents with slowness of movement, difficulty buttoning his shirt, and reduced arm swing while walking. His wife says he takes "short, shuffling steps." He has a resting tremor in his right hand. On examination, there is cogwheel rigidity. What is the most likely underlying pathology?

الإجابة: C. Degeneration of dopaminergic neurons in the substantia nigra
Bradykinesia, resting (pill-rolling) tremor, cogwheel rigidity and a shuffling gait with reduced arm swing = Parkinson disease. Its pathology is loss of dopaminergic neurons in the substantia nigra pars compacta (with Lewy bodies).
- Cerebellar neuron loss (A) gives ataxia and an intention tremor, not a resting tremor with rigidity.

A
ليش غلط
Cerebellar disease causes an intention tremor on reaching a target, not the resting tremor with rigidity and bradykinesia seen here.

B
ليش غلط
Parkinson disease is not a white-matter (demyelinating) disease; it involves loss of dopaminergic neurons in the basal ganglia, particularly the substantia nigra.

C
ليش صح
Parkinson disease pathology is loss of dopaminergic neurons in the substantia nigra pars compacta (with depigmentation of the substantia nigra); Parkinsonism 'TRAPs' the patient: tremor, rigidity, akinesia and postural instability.

D
ليش غلط
Parkinson disease is a neurodegenerative condition with progressive loss of dopaminergic neurons, not an autoimmune inflammatory process of synapses.
