الإجابة: C. N-acetylcysteine
This is paracetamol (acetaminophen)-induced acute liver failure: jaundice, encephalopathy, very high transaminases and INR 5. IV N-acetylcysteine refills liver glutathione and has been shown to reduce mortality in paracetamol-related ALF, even late in the course.
- FFP and vitamin K (B, D) only correct the INR. They do not improve survival, and the INR is needed to track prognosis.
- Activated charcoal alone (A) is only useful very early after ingestion.
