الإجابة: B. SABA
The first step in an acute exacerbation is an inhaled short-acting β2-agonist (SABA), the rapid-onset bronchodilator that is the mainstay of emergency treatment. ICS are controllers. They are not the initial rescue therapy in an exacerbation; systemic steroids are added after the SABA.

A
ليش غلط
Inhaled corticosteroids are controllers: used regularly they reduce airway hyperresponsiveness and exacerbation frequency. They are not the rescue treatment for an acute attack.

B
ليش صح
An inhaled β2-agonist is first-line therapy in an acute exacerbation: a SABA (e.g. albuterol) is the rescue drug for acute attacks, acting within 2 to 5 minutes.
