الإجابة: D. Stress test
A 50-year-old with chest pain and a normal resting ECG has an intermediate probability of CAD, and the cohort sources make the exercise stress ECG the investigation of choice to confirm/exclude inducible ischaemia (it is most sensitive when the resting ECG is normal). In the sources, CT/invasive coronary angiography is reserved for inconclusive non-invasive tests or high-risk/refractory patients, and high-sensitivity troponin detects acute myocardial injury, not stable ischaemia. The old bank key (CT coronary angiography) is not supported by the cohort sources.

A
ليش غلط
Coronary angiography follows a positive stress test; it is not the first test to rule ischemia in or out.

B
ليش غلط
CT coronary angiography is done in symptomatic patients with inconclusive non-invasive tests, particularly those with a poor prognosis, so it is not the first test here.

C
ليش غلط
Troponin rises 3-6 hours after myocardial injury and peaks at about 36 hours; it detects infarction in acute syndromes, not exertional ischemia in a stable patient.

D
ليش صح
The exercise ECG is the investigation of choice and the confirmatory test for ischemia in suspected stable angina with a normal resting ECG.
