الإجابة: D. Mitral regurgitation
Days after an MI, sudden pulmonary oedema and hypotension with a new harsh pansystolic murmur at the apex radiating to the axilla = acute mitral regurgitation from papillary muscle rupture. Aortic regurgitation is diastolic, AS is an ejection systolic murmur at the base, and MS is a diastolic rumble.

A
ليش غلط
Aortic regurgitation gives a diastolic decrescendo murmur at the left sternal border with a widened pulse pressure, not an apical pansystolic murmur.

ليش غلط
Aortic stenosis gives a harsh crescendo-decrescendo systolic murmur in the second right intercostal space radiating to the carotids, not an apical murmur radiating to the axilla.

C
